Vet Care Release

We know how important your furry family member is to you. We’re pet people, too, and it's important to us to keep them healthy while in our care. This vet care release is in place to ensure that your pet has access to the best care in the case of an accident or illness.


Client Responsibilities:

As a client of The Dog’s Pajamas (TDP, pet sitter, us, we), you:


*Understand that your pet sitter works hard to prevent accidents and injuries and that such problems may occur no matter how well a pet is cared for. 


*Agree to keep your contact information, your secondary contact, your emergency contact, and your veterinarian information up to date. TDP will make every effort to contact you, your secondary contact, or your emergency contact before veterinary care is sought out.


*Authorize your pet sitter to seek veterinary care from your vet listed in your client portal if your pet becomes ill, injured, or at significant risk of experiencing a medical problem while in the care of TDP.  If your veterinarian is not available, TDP will take your pet(s) to an appropriate clinic.


*Agree to allow your pet sitter to use their best judgment in handling these situations and understand that your pet sitter assumes no responsibility for the actions and decisions of the veterinary staff, the health, or the death of your pet.


*Authorize TDP and the veterinarian caring for your pet(s) to share all medical records with emergency vet clinics to provide the best care possible.


*Assume full responsibility for the payment and/or reimbursement for any veterinary services rendered, including but not limited to diagnosis, treatment, medical supplies, and boarding.


TDP Responsibilities:

As your pet sitter, we agree to:


*Make every effort to prevent accidents and be proactive with your pet’s health. 


*Make every effort to contact you, your secondary contact, and your emergency contact regarding any treatments, illness, injury, or potential problems as soon as the condition is deemed not life-threatening and/or contact is possible. 


This agreement is valid from this date forward for all pets in the care of TDP and grants permission for future veterinary care without additional authorization each time TDP cares for your pet and applies to all of your pets within the care of TDP.
